Extrafine triple therapy and asthma exacerbation seasonality: TRIMARAN and TRIGGER post hoc analyses

چکیده
BackgroundPrevious studies have shown seasonal variation in asthma exacerbations, peaking over the winter months. A single-inhaler triple therapy containing extrafine formulations of inhaled corticosteroid (ICS) beclomethasone dipropionate (BDP), long-acting β2-agonist formoterol fumarate (FF), and muscarinic antagonist glycopyrronium (G) is development for asthma.ObjectiveWe sought to evaluate whether calendar season impacted relative effect BDP/FF/G versus BDP/FF on moderate severe exacerbations.MethodsTRIMARAN TRIGGER were double-blind 52-week comparing with (TRIMARAN medium-dose ICS; high-dose) adults uncontrolled (Asthma Control Questionnaire-7 score ≥1.5), prebronchodilator FEV1 less than 80% predicted, history 1 or more exacerbation, who had been receiving ICS/long-acting at least 4 weeks before entry. Moderate exacerbations captured throughout each study. In these post hoc analyses, annual exacerbation rate was calculated month, ratios determined from events grouped by season.ResultsIn patients received alone, there a marked occurrence highest However, addition component reduced this variation, especially during winter, such that greatest (significant 20.3% reduction [P = .0008]). Reductions other seasons ranged between 8.6% 12.0%.ConclusionsThese analyses indicate reduces peaks confirm overall utility adding management asthma. Previous We exacerbations. Extrafine inhaled corticosteroid therapy in the control of asthma
Small airways disease plays an important role in the pathogenesis of asthma, but assessment of small airways impairment is not easy in everyday clinical practice. The small airways can be examined by several invasive and noninvasive methods, most of which can at present be used only in the experimental setting. Inhalers providing extrafine inhaled corticosteroid particle sizes may achieve suffi...
متن کاملAsthma outcomes and costs of therapy with extrafine beclomethasone and fluticasone.
BACKGROUND Characteristics of inhaled corticosteroids (ICSs) differ, but data comparing the real-life effectiveness of various ICSs for asthma are lacking. OBJECTIVE We sought to compare real-life asthma outcomes and costs of extrafine hydrofluoroalkane (HFA)-beclomethasone and fluticasone administered through a pressurized metered-dose inhaler. METHODS This retrospective matched cohort stu...
متن کاملPremenstrual exacerbation of asthma.
Questionnaires and twice daily peak expiratory flow measurements were used to evaluate the effects of the menstrual cycle in asthmatic women. Forty per cent of women reported premenstrual deterioration in their symptoms and this was confirmed by peak flow recordings. No correlations were found between premenstrual exacerbation of asthma and symptoms of premenstrual tension, consumption of aspir...
متن کاملTriple therapy in COPD: new evidence with the extrafine fixed combination of beclomethasone dipropionate, formoterol fumarate, and glycopyrronium bromide
The goals of COPD therapy are to prevent and control symptoms, reduce the frequency and severity of exacerbations, and improve exercise tolerance. The triple combination therapy of inhaled corticosteroids (ICSs), long-acting beta2 agonists (LABAs), and long-acting muscarinic antagonists (LAMAs) has become an option for maintenance treatment of COPD and as a "step-up" therapy from single or doub...
